Laser eye surgery has no long-term effects on cornea
Washington, Nov 10 : A research team, including an Indian-origin boffin, has found that laser eye surgery that corrects vision does not lead to later problems with the cornea – at least not after nine years.
Two types of laser surgery—photorefractive keratectomy (PRK) and laser in situ keratomileusis (LASIK)—are often used to correct refractive errors such as nearsightedness.
However, little is known about how these procedures affect the cornea, the transparent membrane covering the eye, on the cellular level over the long term.
Sanjay V. Patel, M.D., and William M. Bourne, M.D., of Mayo Clinic, Rochester, Minn., studied 29 eyes of 16 patients who had undergone LASIK or PRK. Photographs of the cells lining the cornea (endothelial cells) were taken and analysed before and nine years after surgery.
The annual rate of corneal endothelial cell loss in the eyes of patients who had had surgery was compared with those of 42 eyes that had not undergone either procedure.
Nine years after surgery, the density of cells lining the cornea had decreased by 5.3 percent from their preoperative state.
However, the average annual rate of cell loss (0.6 percent) was the same in corneas of eyes that were operated on and those that were not.
“Our results support the findings of numerous short-term studies that found no significant endothelial cell loss after LASIK and PRK,” the authors said.
They added: “The importance of the findings in our study relates to using corneas that have undergone LASIK or PRK as donor tissue.”

How Much Are Laser Eye Treament Costs?
Laser eye surgery is a permanent procedure to alter your cornea with the ultimate aim of improving your vision so that you rely less or not at all on prescription glasses or contact lenses. Another treatment is known as photreactive keratectomy or PRK. This involves no cutting a flap into the cornea at all. Instead, a very small layer is scraped off altogether before a laser is used. Don’t worry, it’s not as bad as it sounds; that small layer grows back itself.
The costs of any one of these treatments can vary greatly depending on what surgeon you use and the technology involved. On average you can expect to pay anywhere from $1,500 to $3,500 per eye. The downside is most health insurers don’t cover it, but having said that most people who have the treatment only sing its praises.

Intraocular melanoma is a disease in which malignant (cancer) cells form in the tissues of the eye.
Intraocular melanoma begins in the middle of 3 layers of the wall of the eye. The outer layer includes the white sclera (the "white of the eye") and the clear cornea at the front of the eye. The inner layer has a lining of nerve tissue, called the retina, which senses light and sends images along the optic nerve to the brain.
The middle layer, where intraocular melanoma forms, is called the uvea or uveal tract, and has 3 main parts:
- Iris
The iris is the colored area at the front of the eye (the "eye color"). It can be seen through the clear cornea. The pupil is in the center of the iris and it changes size to let more or less light into the eye.
- Ciliary body
The ciliary body is a ring of tissue with muscle fibers that change the size of the pupil and the shape of the lens. It is found behind the iris. Changes in the shape of the lens help the eye focus. The ciliary body also makes the clear fluid that fills the space between the cornea and the iris.
- Choroid
The choroid is the layer of blood vessels that bring oxygen and nutrients to the eye. Most intraocular melanomas begin in the choroid.

Intraocular melanoma is a rare cancer, but it is the most common eye cancer in adults.

Anything that increases your risk of getting a disease is called a risk factor. Having a risk factor does not mean that you will get cancer; not having risk factors doesn’t mean that you will not get cancer. People who think they may be at risk should discuss this with their doctor. Risk factors for intraocular melanoma include the following:
- Older age.
- Being white.
- Having a fair complexion (light skin) or green or blue eyes.
- Being able to tan.
Possible signs of intraocular melanoma include a dark spot on the iris or blurred vision.
Intraocular melanoma may not cause any early symptoms. It is sometimes found during a routine eye exam when the doctor dilates the pupil and looks into the eye. The following symptoms may be caused by intraocular melanoma or by other conditions. A doctor should be consulted if any of these problems occur:
- A dark spot on the iris.
- Blurred vision.
- A change in the shape of the pupil.
- A change in vision.
Glaucoma may develop if the tumor causes the retina to separate from the eye. If this happens, there may be no symptoms, or symptoms may include the following:
- Eye pain.
- Blurred vision.
- Eye redness.
- Nausea.
Tests that examine the eye are used to help detect (find) and diagnose intraocular melanoma.
The following tests and procedures may be used:
- Eye exam with dilated pupil: An examination of the eye in which the pupil is dilated (enlarged) with medicated eyedrops to allow the doctor to look through the lens and pupil to the retina. The inside of the eye, including the retina and the optic nerve, is examined using an instrument that produces a narrow beam of light. This is sometimes called a slit-lamp exam. The doctor may take pictures over time to keep track of changes in the size of the tumor and how fast it is growing.
- Indirect ophthalmoscopy: An examination of the inside of the back of the eye using a small magnifying lens and a light.
- Ultrasound exam of the eye: A procedure in which high-energy sound waves (ultrasound) are bounced off the internal tissues of the eye to make echoes. Eye drops are used to numb the eye and a small probe that sends and receives sound waves is placed gently on the surface of the eye. The echoes make a picture of the inside of the eye. The picture, called a sonogram, shows on the screen of the ultrasound monitor.
- Transillumination of the globe and iris: An examination of the iris, cornea, lens, and ciliary body with a light placed on either the upper or lower lid.
- Fluorescein angiography: A procedure to look at blood vessels and the flow of blood inside the eye. An orange fluorescent dye (fluorescein) is injected into a blood vessel in the arm. As the dye travels through blood vessels of the eye, a special camera takes pictures of the retina and choroid to detect any blockage or leakage.

Pterygium and Pinguela
A Pterygium is fleshy tissue that grows over the cornea (the clear front window of the eye). It may remain small or may grow large enough to interfere with vision. A pterygium most commonly occurs on the inner corner of the eye, but can appear on the outer corner as well. The exact cause is not well understood. Pterygium occurs more often in people who spend a great deal of time outdoors, especially in sunny climates. Long-term exposure to sunlight, especially ultraviolet (UV) rays, and chronic eye irritation from dry, dusty, smoky conditions seem to play an important causal role. A dry eye may contribute to pterygium.
When a pterygium becomes red and irritated, eyedrops or ointments may be used to help reduce the inflammation. If the pterygium is large enough to threaten sight or grows rapidly, it can be removed surgically. A pterygium may also cause decreased vision by causing astigmatism. It is therefore advisable to follow up on medium to large sized pterygia at least yearly and include a refraction.
Despite proper surgical removal, the pterygium may return, particularly in young people. Protecting the eyes from excessive ultraviolet light with proper sunglasses and avoiding dry, dusty conditions and use of artificial tears may also help.
A Pinguecula is a yellowish patch or bump on the white of the eye, most often on the side closest to the nose. It is not a tumor, but an alteration of normal tissue resulting in a deposit of protein and fat. Unlike a pterygium, a pinguecula does not actually grow onto the cornea. A pinguecula may also be a response to chronic eye irritation or sunlight.
No treatment is necessary unless it becomes inflamed. A pinguecula does not grow onto the cornea or threaten sight. If particularly annoying, a pinguecula may on rare occasions be surgically removed, but the postoperative scar may be as cosmetically objectionable as the pinguecula.
